What can help a sore knee without surgery?

A steady knee ache often starts with gentle movement and strength work. A cane, brace or medicine may help when it suits the cause. Shorter walks or lighter chores can give the knee time to settle. If it becomes hot, locks or won't hold weight, get prompt medical care.

Is walking useful when a knee has bone-on-bone arthritis?

Walking can remain useful when the distance suits the knee. Turn back before you start to limp, shorten a step or walk differently. Use a steady surface and add distance slowly. If walking, sleep and chores keep getting harder, discuss surgery instead of repeating care that hasn't helped.

Can a person keep golfing with knee arthritis?

Many people keep part or all of a round by changing how much the knee does. A cart cuts the walking, while shorter practice means fewer turns. Strength and balance work may support the game. More swelling, giving way or lost motion means the knee needs another exam.

How often can cortisone be used in a knee?

There isn't one safe number for everyone. The answer depends on the cause, health history and how earlier shots worked. Relief may be short, and repeat use has risks. Ask what the cortisone is meant to ease, how long relief may last and when the knee will be checked again.

When is knee replacement worth discussing?

Replacement deserves discussion when the worn area seen on the X-ray is also where the knee hurts. Walking, sleep, stairs and basic chores may keep getting harder even after other care. A surgeon can review the exam, health risks and recovery. The talk doesn't commit you to surgery.

How much do joint procedures cost in Arizona?

The Cochrane review of land-based therapeutic exercise for knee osteoarthritis extracted data from 54 randomised trials, assessing pain, physical function and quality of life immediately after treatment and the sustained effect at 2-6 months and beyond 6 months. Only 19 of the included studies (20%) met all three low-risk-of-bias criteria the authors applied.

A systematic review and meta-analysis of 120 randomised trials (10,253 participants) covering resistance training across the knee osteoarthritis continuum - 88 trials in early OA, 13 preoperative, 19 after knee replacement - found improvements in mobility, walking capacity and knee extension strength in early OA (SMD 0.46-0.81, moderate-to-high GRADE), in preoperative knee extension strength (SMD 0.47, high GRADE) and in mobility after knee replacement (SMD 0.58). Resistance training improved pain, symptoms, function and quality of life in early OA but showed NO significant effect on those outcomes preoperatively, with no increased risk versus controls at any stage.

The 2023 ACR/AAHKS timing guideline conditionally recommends AGAINST delaying hip or knee arthroplasty to pursue additional non-operative treatment - physical therapy, NSAIDs, ambulatory aids or intra-articular injections - in patients with moderate-to-severe osteoarthritis for whom non-operative therapy has already been ineffective and who have chosen surgery. It conditionally recommends delay for nicotine cessation and for better glycemic control in diabetes, states that obesity by itself is not a reason for delay while weight loss should be strongly encouraged, and conditionally recommends against delay in patients with severe deformity or bone loss. Evidence for all recommendations was graded low or very low quality.

FDA states directly that regenerative medicine therapies - including stem cells, stromal vascular fraction, umbilical cord blood, amniotic fluid, Wharton's jelly, ortho-biologics and exosomes - have NOT been approved for the treatment of any orthopedic condition, naming osteoarthritis, tendonitis, disc disease, tennis elbow, back pain, hip pain, knee pain, neck pain and shoulder pain. FDA further states that being charged for these products, or being offered them outside an FDA-overseen clinical trial, means a patient is likely being deceived and offered a product illegally, and that a product's presence on clinicaltrials.gov or a firm's FDA registration does not mean the product is legally marketed. Reported harms include blindness, tumor formation, neurological events and life-threatening bacterial infections.

The Cochrane review of land-based therapeutic exercise for HIP osteoarthritis included 10 randomised trials, seven of which the authors judged to have a low risk of bias, though results remain vulnerable to performance and detection bias because participants could not be blinded and all primary outcomes were self-reported. The evidence base for hip is substantially thinner than for knee.
